HOSPITAL SERVICE DISTRICT NO. 3
The following resolution was offered for adoption by Dr. Matthew Courville and seconded by Paula Manuel:
SECOND SUPPLEMENTAL RESOLUTION
A resolution authorizing the extension of the maturity on the Taxable Bond Anticipation Notes, Series 2022, of Hospital Service District No. 3 of the Parish of Allen, State of Louisiana; and providing for other matters in connection therewith.
WHEREAS, Hospital Service District No. 3 of the Parish of Allen, State of Louisiana (the “Issuer”), has heretofore issued its Taxable Bond Anticipation Notes, Series 2022, dated October 6, 2022 (the “Notes”), pursuant to a resolution adopted by the Board of Commissioners of Hospital Service District No. 3 of the Parish of Allen, State of Louisiana (the “Governing Authority”), acting as governing authority of the Issuer, on May 16, 2022, as amended by a resolution adopted on August 1, 2022, and a resolution adopted on August 16, 2022 (the “Amended and Restated Bond Resolution”); and
WHEREAS, this Governing Authority adopted a supplemental resolution on August 8, 2024, authorizing the extension of the maturity on the Notes to not later than April 6, 2025; and
WHEREAS, this Governing Authority now desires to adopt this resolution to provide for a second extension of the maturity of the Notes to not later than July 6, 2025;
NOW, THEREFORE, BE IT RESOLVED by the Governing Authority, acting as the governing authority of the Issuer, that:
SECTION 1. Subject to the approval of the State Bond Commission and the Police Jury of the Parish of Allen, State of Louisiana, approval is hereby given to the extension of the maturity date of the Notes to not later than July 6, 2025.
SECTION 2. Application is hereby made to the State Bond Commission, Baton Rouge, Louisiana, for approval of the extension of the maturity on the Notes to not later than July 6, 2025. A certified copy of this resolution shall be submitted to the State Bond Commission together with a request for prompt consideration and approval of this application.
By virtue of applicants/issuer’s application for, acceptance and utilization of the benefits of the Louisiana State Bond Commission’s approval(s) resolved and set forth herein, it resolves that it understands and agrees that such approval(s) are expressly conditioned upon, and it further resolves that it understands, agrees and binds itself, its successors and assigns to, full and continuing compliance with the “State Bond Commission Policy on Approval of Proposed Use of Swaps, or other forms of Derivative Products Hedges, Etc.”, adopted by the Commission on July 20, 2006, as to the borrowing(s) and other matter(s) subject to the approval(s), including subsequent application and approval under said Policy of the implementation or use of any swap(s) or other product(s) or enhancement(s) covered thereby.
SECTION 3. Application is hereby made to the Police Jury of the Parish of Allen, State of Louisiana, for consent and authority to the extension of the maturity on the Notes to not later than July 6, 2025. A certified copy of this resolution shall be forwarded to said Police Jury on behalf of the Issuer.
SECTION 4. The Executive Officers (as defined in the Amended and Restated Bond Resolution) are hereby empowered, authorized and directed to take any and all action and to execute and deliver any instrument, document or certificate necessary to effectuate the extension of the maturity on the Notes as contemplated by this resolution, including but not limited to accepting one or more consents to the extension authorized herein.
SECTION 5. A copy of this resolution shall be published immediately after its adoption in one issue of the official journal of the Issuer.
SECTION 6. All other resolutions or ordinances or parts thereof in conflict herewith are hereby repealed to the extent of such conflict.
SECTION 7. If any provision of this resolution shall be held to be illegal or invalid, such illegality or invalidity shall not affect any other provision of this resolution, but this resolution shall be construed and enforced as if such illegal or invalid provisions had not been contained herein. Any constitutional or statutory provision enacted after the date of this resolution which validates or makes legal any provision of this resolution which would not otherwise be valid or legal, shall be deemed to apply to this resolution.
SECTION 8. This resolution shall become effective immediately upon its adoption.
This Resolution having been submitted to a vote, the vote thereon was as follows:

The Allen Parish Police Jury met in regular session at the Police Jury Administration Building, Oberlin, Louisiana, February 3, 2025 at the hour of 6:00pm.

The Lord’s Prayer was said followed by the Pledge of Allegiance.
Held a public hearing to abandon 1300’ of the north end of Benjamin Lane off of Hwy 26 in Road District 4.
Motion seconded and carried to adopt an ordinance to abandon 1300’ of the north end of Benjamin Lane off of Hwy 26 in Road District 4.
Motion seconded and carried to approve minutes of the meetings held January 6, 2025.
The CO2 Committee had the following recommendations:

The Finance Committee had the following recommendations:
1) Approve to pay Prestige Plumbing Repair $5,300.00 to repair a back flow preventor and inspecting test at the Courthouse.
2) Approve to pay Sirius Power Generation $5,870.47 to replace the generator control panel at the DA’s office.
3) Approve to pay Ray’s Construction $2,975.00 to stain and repair base trim inside the Courthouse.
4) Approve to pay Cotton’s Heating & Cooling $16,563.00 to replace the unit on the roof of the Registrar of Voters office.

The Road Committee had the following recommendations:
1. Approve a JSA with the City of Oakdale for grass cutting, pothole patching and trash pick-up for the month of February, 2025.
2. Approve a JSA with Allen Parish School Board for manpower, equipment and material to repair the following bus turnarounds:

1268 Simmons Road in Fairview
End of Dallas Road in Oberlin
3. Approve a JSA with Fire District #5 (Ward 1) for 1 load of rock at the Castor Fire Station driveway.
4. Approve a Joint Service Agreement with the Town of Kinder to replace the Park Road Bridge utilizing grant funds from the State of Louisiana and for the Town of Kinder to pay the Contractor through the Allen Parish Police Jury for Additive Alternate No. 1 and Additive Alternate No. 2.

Recognized Mrs. Sonnier to present the monthly inmate report. Expenses for the month of January 2025 totaled $18,553.48. We are under budget $4,846.52 for the year. Currently there are 120 Parish and 17 DOC inmates.

Notice is hereby given that, in accordance with L.R.S. 3:1609 and LAC 7:XV.314 (A), the Louisiana Department of Agriculture & Forestry, Louisiana Boll Weevil Eradication Commission, has established a boll weevil eradication zone, the Louisiana Eradication Zone, consisting of all the territory within the state of Louisiana.
Notice is further given that all producers of commercial cotton in Louisiana are required to participate in the boll weevil eradication program, including cost sharing, in accordance with the Boll Weevil Eradication Law and regulations. This includes, but is not limited to, reporting of cotton acreage and destruction of cotton plants and stalks by December 31 of each crop year. Pursuant to the provisions of a resolution adopted by the Board of Commissioners of Hospital Service District No. 3 of the Parish of Allen, State of Louisiana (the “Governing Authority”), acting as the governing authority of Hospital Service District No. 3 of the Parish of Allen, State of Louisiana (the “District”), on December 17, 2024, NOTICE IS HEREBY GIVEN that a special election will be held within the District on SATURDAY, MAY 3, 2025, and that at the said election there will be submitted to all registered voters in the District qualified and entitled to vote at the said election under the Constitution and Laws of the State of Louisiana and the Constitution of the United States, the following proposition, to-wit:
