Allen Parish Ambulance Service Board of Directors met for monthly meeting at the Allen Parish Ambulance Service Office on Tuesday, April 9, 2024 at 4:30p.m.
Members Present: Lowell Keys, Al Ritter, Sarah Ryland, Chuck Welch and Sharon Grundy.
Chairman Keys called the meeting to order promptly at 4:30 p.m. The Pledge of Allegiance was said by all.
Motion by Ritter, seconded by Ryland to approve and dispense with the reading of the minutes from the March 12, 2024 board meeting. Motion passed.

Motion by Ryland, seconded by Grundy to approve opening new COs. Motion passed.
Motion by Ritter, seconded by Welch to approve (1) board write off in the amount of $474.56. Motion passed.
Motion by Grundy, seconded by Ryland to submit (1) bad debt account to collections in the amount of $654.89. Motion passed.
Motion by Welch, seconded by Ryland to approve Policy and Procedure updates. Motion passed.
Executive Director Istre gave the Fleet Report and Financial Report. Istre discussed financials and reviewed budget variances with the Board. Motion by Ritter, seconded by Grundy to amend the 2024 budget. Motion passed.
Motion by Ryland, seconded by Welch to adjourn. Motion passed.

Bona fide bidders may obtain a CD of the proposed contract documents in PDF format, including plans and specifications, from King Architects, Inc. at 1312 Sampson St., Westlake, LA 70669 for $25.00 per each set of documents on CD. Or complete physical, paper documents may be obtained upon deposit of $200.00 for each set of documents. Deposit is fully refundable for the first set of documents to all bona fide prime* bidders upon return of the documents in good condition no later than ten (10) days after receipt of bids. Prime bidders are defined as licensed General Contractor bidding this job as such.
Preference is given to materials, supplies, and provisions that are produced manufactured, or grown in Louisiana, quality being equal to articles offered by competitors outside the State.
A Non-Mandatory Pre-Bid Conference for general contractors with the Project Architect will be held at the project site on Monday, May 20th, 2024, at 10:00 A.M. Bidders must be present the entire meeting in order to bid.
All bids must be accompanied by bid security equal to five percent (5%) of the sum of the base bid and all alternates, and must be in the form of a certified check or cashier’s check drawn on a bank insured by the FDIC, or the Bid Bond Form contained in the bidding documents, which shall be written by a surety or insurance company currently on the U.S. Department of the Treasury Financial Management Service list of approved bonding companies which is published annually in the Federal Register or by a Louisiana domiciled insurance company with at least an A- rating in the latest printing of the A.M. Best’s Key Rating Guide to write individual bonds up to ten percent (10%) of policyholders’ surplus as shown in the A.M. Best’s Key Rating Guide or by an insurance company in good standing licensed to write bid bonds which is either domiciled in Louisiana or owned by Louisiana residents, all in accordance with LSA-R.S. 38:2218. No Bid Bond indicating an obligation of less than five percent (5%) by any method is acceptable.
The successful bidder shall be required to furnish a Performance and Payment Bond written by a company licensed to do business in Louisiana, in an amount equal to 100% of the Contract amount, written by a surety company licensed to do business in Louisiana with an A.M. Best Rating of “A-” or better, all in accordance with LS R.S. 38:2219. The bond shall be countersigned by a person who is under contract with the surety company or bond issuer as an insurance agent in this State, and who is residing in this State.
Bids shall be accepted only from contractors who are licensed under LS R.S. 37:2150-2163 for the classification of Building Construction. No bid may be withdrawn for a period of forty-five (45) days after receipt of bids, except under the provisions of LS R.S. 38:2214.
The Owner reserves the right to reject any and all bids for just cause. In accordance with LS R.S. 38:2212(B)(1), the provisions and requirements of this Section, and those stated in the Bidding Documents shall not be waived by any entity. Awarding or rejecting bids shall be in accordance with LA Public Bid Law Statues.
The public shall incur no obligation to the Contractor until the Contract between Lognion Investments, LLC and the Contractor is fully executed.
Official action on this bid will be taken within forty-five (45) days by VILLIAGE OF REEVES.

The Allen Parish Police Jury is seeking a vendor to supply a modular steel bridge structure only for the replacement of the Park Road Bridge. The construction of abutments and installation of the bridge will be bid under separate contract.
Complete specifications for this project are available in electronic form only. They may be obtained without charge and without deposit from the Allen Parish Engineer, Jacob Dillehay, P.E. at jdillehay@appj.us. Printed copies are not available from the Designer, but arrangements can be made to obtain them through most reprographic firms. Plan holders are responsible for their own reproduction costs. The Police Jury of Allen, Louisiana, will receive sealed bids up to the hour of 4:00 p.m., June 3, 2024, at the Allen Parish Police Jury Administration Building, in Oberlin, Louisiana, for the purchasing of the following materials:
1. Creosote bridge timbers/materials, and pilings, S1S1E, 1 Olb treatment.

7. Hot Mix Asphalt and Cold Mix: Per ton FOB and delivered to various barns.
8. Furnish equipment and labor to lay hot mix asphalt for small projects.
All bids, will be for a six month period beginning July 1st through December 31st 2024 unless otherwise specified. Bids will be opened on June 3rd at 4:00PM and announced at the public meeting at 6:00.

For Environmental Risk Assessment Services
The Parish is soliciting proposals from qualified firms to provide Environmental Risk Assessment Services. The qualified firm shall be responsible for performing a study and evaluation to report all potential environmental risks associated with CO2 sequestration projects in Allen Parish.
MANDATORY PRE- RFQ SUBMITTAL MEETING: A mandatory, in-person meeting will be held on May 30th, 2024 at 2:00 p.m. at the Allen Parish Police Jury Office, 602 Court Street, Oberlin, LA 70655.
Failure for a representative from the firm to attend this meeting in person will result in disqualification from the RFQ process and the firm’s RFQ will be given no further consideration.

Meeting April 15, 2024

The meeting was called to order by President Logan Hamilton at 5:55 p.m. He led the Board in prayer and Pledge of Allegiance was said by all.
Katrena motioned to accept the previous meeting minutes as read and April seconded the motion. Motion carried.
Steven DeRouen with Steven DeRouen & Associates attended the meeting. He presented the 2023 Annual Audit for the Water District to the Board. He explained financials and specific audit procedures. Overall he stated that the audit went well.
Adam updated the Board on the Wells. Well 3 generator is ordered. The thunderstorm last week caused damage at Well 3 which resulted in burning up the controls on the switch gear. Asco Power Services is coming tomorrow to repair it.
Well 5 is back together and running. Chlorinated and sample will be pulled tomorrow. We be back online after sample clears.
Griner Drilling Service will pull apart, inspect, backwash and perform a pump test on Well 6.
The Board reviewed & discussed the Financial Report. Reggie made a motion to accept the Financial Report and pay all bills submitted and Katrena seconded the motion. Motion carried.
Adam reported on the water system. The Prison lift pump failed and it will cost $29,895.00 to be repaired.
Well 4 motor failed. Griner Drilling Service will tear down and inspect to determine the issue.
Gary made a motion to adjourn the meeting at 6:51 p.m. and April seconded the motion. Motion carried.
