Request for Statements of Qualifications A/E Services
The Oakdale Housing Authority is accepting proposals from architects/engineers for services required in its 2023 CFP grant funded by HUD until noon, February 2, 2024.
The work will be funded by HUD. Work includes preparation of plans, specifications, contract documents, and construction inspection, etc. on improvements of dwelling units.
The statement of qualifications should contain a letter of interest and resume addressing the firm’s ability to perform work, firm’s staff, firm’s prior experience and references.
The Oakdale Housing Authority Commissioners will evaluate responses on basis of these criteria and rank in numerical order: (1) evidence of firm’s ability to perform work: 0-10, (2) capability to provide services in a timely manner: 0-10, (3) A/E registered in state to do work: 0-10, (4) prior experience with the Oakdale Housing Authority: 0-10, (5) ability to meet schedules: 0-10, (6) knowledge of local codes: 0-10, (7) statement that firm is not debarred by HUD or other federal agency: 0-10.

Proposals shall be addressed to the Village of Elizabeth and delivered to the office of the Engineer, Meyer, Meyer, LaCroix & Hixson, Inc. located at 100 Engineer Place, Alexandria, LA 71303 not later than 10:00 A.M., local time on February 8, 2024. Bidding documents on a USB flash drive may be obtained from the office of the Engineer at a cost per copy of $40.00, which represents the cost of reproduction and handling, and is nonrefundable. Any requests for bid documents must be accompanied by payment in full. Prime Bidders must obtain Bidding Documents from the Engineer. Bids received from Contractors utilizing any other Bidding Document source will be returned unopened. Each bidder must have an active Unique Entity ID (UEI) (SAM), as verified on www.sam.gov, prior to the beginning of construction.
Each bid must be accompanied by bid security made payable to Owner in an amount of 5 percent of bidder’s maximum bid price and in the form of a certified check, cashier’s check or a bid bond. The successful bidder will be required to obtain a Performance Bond and a Payment Bond each in the amount of 100% of the contract amount. All bonds shall be executed by such sureties as are named in U.S. Department of the Treasury Circular 570. A bond signed by an agent or attorney-in-fact must be accompanied by a certified copy of that individual’s authority to bind the surety.
Bidders may not withdraw their bid within forty-five (45) days after the actual date of opening thereof. The Owner reserves the right to reject any and all bids for just cause in accordance with Louisiana R.S. 38:2214B.
The Contractor shall begin mobilization and procurement of materials within ten (10) working days of the receipt of the Notice to Proceed.
This contract is subject to the requirements of the Davis-Bacon Act (DOL 29 CFR Part 5), as amended. The Contractor is required to comply with wage and labor provisions and to pay minimum wages in accordance with the schedule of wage rates established by the United States Department of Labor. The attention of Bidders is called particularly to the requirements for conditions of employment to be observed and minimum wage rates to be paid under the Contract, Section 3, Segregated Facilities, Section 109, Executive Order 11246, and all applicable laws and regulations of the Federal government and State of Louisiana and bonding and insurance requirements.

IMPORTANT INFORMATION ABOUT YOUR DRINKING WATER
City of Oakdale Water System failed to Take Corrective Action Following Identification of a Significant Deficiency
On 04/20/2023 the Louisiana Department of Health (LDH) performed a detailed inspection and engineering evaluation of our water system called a sanitary survey. During the sanitary survey, they identified one or more significant deficiencies in our system. As our customers, you have a right to know what happened and what we are doing to correct this situation. According to EPA’s Ground Water Rule, we were required to correct the deficiency (or deficiencies) or work with the State to develop a plan to correct the deficiency (or deficiencies). However, we failed to correct the deficiency or contact the State before the established deadline and have therefore violated a requirement of the Ground Water Rule.
What should I do?
There is nothing you need to do. You do not need to boil your water or take other corrective actions. If a situation arises where the water is no longer safe to drink, you will be notified within 24 hours.
What does this mean?
This is not an emergency. If it had been an emergency, you would have been notified within 24 hours.
This significant deficiency has the potential to result in lack of proper treatment and oversight of the water system. Inadequately treated or inadequately protected water may contain disease-causing organisms. These organisms can cause symptoms such as diarrhea, nausea, cramps, and associated headaches. While we have not detected any evidence of contamination or other health threats related to our source water, we are still committed to correcting the deficiency to eliminate the threat of contamination .
What was done?
A Cross-Connection Control Plan was implemented and delivered to customers that needed their backflow prevention devices tested. All customers have completed this with the exception of Family Dollar and Martco but are in the process of completing theirs as well.

NOTICE OF SPECIAL ELECTION
Pursuant to the provisions of a Resolution adopted on September 11, 2023, by the Parish School Board of the Parish of Allen, State of Louisiana (the “Governing Authority”), acting as the governing authority of School District No. 25 of Allen Parish, State of Louisiana (the “District”), NOTICE IS HEREBY GIVEN that a special election will be held in the District on SATURDAY, MARCH 23, 2024, and that at said election there will be submitted to all registered voters of the District, qualified in and entitled to vote at said election under the Constitution and laws of the State of Louisiana 1974, as amended, and the Constitution of the United States and by other constitutional and statutory authority supplemental thereto, the following proposition, to wit:

Shall School District No. 25 of the Parish of Allen, State of Louisiana (the “District”), be authorized to continue to levy a special tax of five and sixty-five hundredths (5.65) mills on all the property subject to taxation within said District (an estimated $169,067.00 reasonably expected at this time to be collected from the levy of the tax for an entire year), for a period of ten (10) years, beginning with the year 2026 and ending with the year 2035, for the purpose of constructing, improving, operating and maintaining school facilities in and for said District, said millage to represent a fourteen-hundredths of a mill (.14) increase over the 5.51 mills tax authorized to be levied through the year 2025 pursuant to an election held on April 9, 2016?
The special election will be held in at the following polling places situated in the District, and which polls will open at 7:00 a.m. and close at 8:00 p.m. on SATURDAY, MARCH 23, 2024, in compliance with the provisions of Section 541 of Title 18 of the Louisiana Revised Statutes of 1950, as amended (La. R.S. 18:541). The Commissioners-in-Charge and the Commissioners authorized to be selected by the Parish Board of Election Supervisors at the special election on SATURDAY, MARCH 23, 2024, are hereby designated as the Commissioners-in-Charge and the Commissioners to serve at the following described polling places designated for this election, to-wit:

The polling places set forth above and situated in the District, be and the same are hereby designated as the polling places at which to hold the special election, and the Commissioners-in-Charge and Commissioners, respectively, shall be those persons designated according to law.
A portion of the monies collected from the Tax shall be remitted to certain state and statewide retirement systems in accordance with the provisions of Subtitle 1 of Title 11 of Louisiana Revised Statutes of 1950, as amended (La. R.S. 11:82).
In accordance with La. R.S. 18:1285(A)(1)(a)(v), the estimated cost of holding the election will be $13,600.00
The special election will be held in accordance with the applicable provisions of Chapter 5 and Chapter 6-A of Title 18 of the Louisiana Revised Statutes of 1950, as amended, and other constitutional and statutory authority as applicable thereto, and the officers appointed to hold the election, or such substitutes therefor as may be selected and designated in compliance with law, will make due returns thereof to the District, and NOTICE IS HEREBY FURTHER GIVEN that the Governing Authority will meet on Monday, April 8, 2024, at 5:00 p.m. at the Allen Parish School Board Office, 1111 West 7th Avenue, Oberlin, Louisiana, and will then and there, in open and public session, proceed to examine and canvass the returns and declare the result of the special election. All registered voters residing in the District are entitled to vote at the special election and voting machines will be used in connection therewith.

d. Associate degree with 60 semester hours
of required qualified courses

4. Ability to provide direct care to students and
assist with feeding, grooming, lifting,
positioning, personal hygiene, toileting and
other activities of daily living.
5. Perform specific procedures including
catheterization, irrigating colostomies, tube
feeding, nasopharyngeal suctioning and
irrigation.
Physical Requirements: A physical exam is required prior to beginning work. The ability to perform all responsibilities with bilateral hand usage, bending, kneeling, lifting, reaching, standing, walking briskly to respond to student needs or emergencies, and pushing a wheel chair. Capacity for heavy lifting of 50 pounds independently and 100 pounds or more with assistance.
Salary: Allen Parish School Board Salary Schedule
Status of Employment: 9 months (180 days)
